Event Registration

The music camp is tuition free for accepted students. To apply,students must have at least one (1) years experience playing their instruments and be able to read standard music notation You will be notified of your acceptance by the camp director. Campers need to be age 10-15. Campers wishing to play in the advanced orchestra must sign up to audition below. There is a $20 enrollment fee due no later than June 8, 2012.

Please submit a separate registration for each camper.

All questions MUST be answered for your registration to be accepted.

Students must be recommended by their music teacher.

If checked yes, Advanced Orchestra auditions will be on May 26 between 10 and 1:00 pm at St. Paul's Episcopal Church and June 9 between 10 and 1:00 pm at St. Paul's Episcopal Church.

Demographic information is gathered for use when applying for funding. This is a key component of determining our level of funding for events such as Strings & Jazzy Things.

If accepted, I will participate fully every day and follow the "Community Standards" and regulations of the school and Strings & Jazzy Things staff and volunteers. I understand that if I do not follow the camp standards, I may be asked to leave the camp.

Community Expectations and Standards

Campers are expected to attend the entire camp experience which includes the final Celebration Concert Friday at 6:00. If you cannot make this commitment, do not enroll in the camp. If you do not fulfill this expectation, you may not be able to attend any future camps. If possible, please do not schedule other appointments during the camp.

No electronics communication devices are allowed to be used at the camp. This included cell phones, I-pads, etc. If campers need to make a call, the S & JT staff will provide a phone. If campers really need to bring a cell phone they must check it in at the registration desk and pick it up at the end of the day. If a camper is found having or using an electronic communication device, it will be confiscated and a parent must retrieve it at a cost of $20. There will be no exceptions to this policy.

  1. You must wear you name-tags at all time during the camp.
    This is for your security and to help the staff and volunteers know who you are.
  2. Strings & Jazzy Things Music Camp are guests of the Broad Ripple facility.
    Each individual is expected to take care of his/ her own things and pick up after themselves.

    A. NO Food or drink is allowed out side of the cafeteria.
    B. If you make a mess clean it up!
    C. Do not go to any other part of the building other than the cafeteria, rehearsal rooms, or auditorium.

  3. Campers, staff, and volunteers will treat each other with dignity and respect.
    Each individual is expected to act and behave appropriately. This means:

    A. No running at any time in the building.
    B. No yelling or loud obnoxious behavior.
    C. No making fun of, picking on, hitting, pinching, poking or taunting others.
    D. Leave other people's property alone.
    E. Obey the directions of staff and volunteers.

  4. You are a guest here. All campers are subject to the discipline process.

    The process is as follows:

    A. 1st incident reported to the camp administrator or Director — Camper is made aware of the infraction and it is kept on record. Camper may have a "time-out."
    B. 2nd incident, the camper will have a "time-out." The parent will be made aware of a problem and advised that the next infraction of community standards will result in the camper being sent home and will be dismissed from the remainder of the camp
    C. 3rd incident will result in the camper being sent home and will be dismissed for the remainder of the camp. The camper may not be admitted to future camps.
    D. Some inappropriate behaviors may constitute an immediate expulsion from the camp as determined by the camp director.
  5. Typing "I AGREE" above will act as your signatures that you have read the Community Standards and understand the expectations and accept these conditions for attending the Strings & Jazzy Things Music Camp.


List up to 2 people who will be authorized to sign in/sign out your child.

IMPORTANT SIGN IN / OUT PROCEDURE: An authorized adult must come inside and sign their child in each morning and sign them out at the end of the day. Children will not be allowed to arrive or leave unattended. SIGN IN: 8:00 a.m. - 9:00 a.m. SIGN OUT: 4:00 p.m. - 5:00 p.m.
 
Please check "yes" if your child has any physical or medical conditions that could affect participation in the camp.
 

In the event of a medical emergency, I give permission to the Strings and Jazzy Things staff to notify family and secure emergency medical attention.
